
Purpose: The purpose of this form is to enforce policies for events or activities that could possibly result in some degree of harm to persons or in some amount of defacement or damage to public or private property (deemed to be potentially hazardous).
All events on campus that meet the definition of a potentially hazardous event including those sponsored by staff, faculty, departments/offices, registered student organizations and outside organization must complete the SAFE Form.
The SAFE Form must be completed and turned into the Office of Student Involvement (OSI) at least 15 calendar days prior to the date of the scheduled event, failure to do so may result in the organization being denied use of university facilities and not be able to conduct the event.
